Omada Health Partners with Intermountain Healthcare and Castell to Establish Integrated Virtual Care Option for Diabetes Management and Prevention

SAN FRANCISCO–(COMMERCIAL THREAD)–Omada Health, a virtual and integrated healthcare provider for chronic care, and Castell, the value-based care subsidiary of Intermountain Healthcare, have entered into a partnership that expands access of the diabetes prevention program and the Omada diabetes to patients and healthcare professionals receiving care from Intermountain Healthcare Primary Care Providers within its Utah Medical Group. Castell will help locate and enroll suitable patients across their existing care pathways. Patients who have or are at risk of diabetes will be proactively contacted by Castell Care Coordinators and Intermountain providers to give them the option to enroll in the appropriate Omada program.

The Diabetes Awareness Program and the Omada Health Diabetes Program offer members support along with the intervention of certified health care educators through the CDC-affiliated Diabetes Training and Technical Assistance Center. Program members receive pre-connected cellular scales, with diabetes program members also receiving continuous glucose monitors and blood glucose meters, all of which report data seamlessly and track progress in real time. Diabetes Prevention and Diabetes programs also include a personalized care plan backed by behavioral science to encourage lasting lifestyle changes.

Diabetes program members experienced an average A1C reduction of 0.8%, including an average 1.4% reduction for individuals with baseline A1C values ​​of 9% or greater, according to the Insights from a digitally enhanced diabetes program she studies.
