NYC Health + Hospitals Debuts Obstetrics Virtual Reality Technology as Part of Its Ongoing and Comprehensive Maternal Health Simulation Courses
Simulation technology allows OB providers to access training courses more easily

NYC Health + Hospitals today announced the introduction of new virtual reality (VR) technology in its library of obstetrics simulation (OB) training courses. Developed in partnership with Health Scholars, a provider of innovative VR health simulations, OB providers in the city’s health system will be able to hone the skills to early detect and successfully address rare but life-threatening health conditions that can arise. during labor and delivery. The new VR technology will integrate training OB providers, including caregivers, midwives, medical assistants and residents, received from in-person training to experience simulated realistic scenarios through a VR headset, enhancing skills and promoting knowledge retention at their convenience during shifts. The new VR OB training technology builds on the municipal health system’s commitment to combat maternal mortality disparities among people of color, often perpetuated by implicit bias.

VR technology transports providers into a labor and delivery suite where they can virtually interact with the patient and team members using real-time voice communications and interactions. They are required to direct the clinical team to perform all appropriate managements according to their hospital’s protocol, allowing them to realistically practice critical cognitive skills such as communication, teamwork, critical thinking and decision making during an emergency. The municipal health system will acquire 22 VR headsets, two for each hospital, where OB providers will be able to check them to complete their training when time permits during shifts. This training will be another arm added to continuing obstetric simulation training and pre- and postnatal maternal care as part of a broader maternal mortality reduction program. The new VR technology will be available to all OB providers within NYC Health + Hospitals in the near future.
The introduction of new VR simulation technology allows OB providers across the healthcare system to more easily access skill training courses in their own time by being able to disconnect from the VR headset and joysticks during downtime during their shifts. Before the introduction of VR technology, entire OB teams had to schedule the time it took to travel to simulation labs through the system to complete individual scenarios.
After disconnecting the VR technology, the OB provider puts on a headset and completes a brief orientation on how to use your voice to interact with virtual team members and manage support. The provider also receives a short description of the patient, as he would receive in real life through the patient record. Transferred to a labor and delivery suite, providers can virtually interact with their patients and team members using real-time voice communications and interactions. They are required to direct the clinical team to perform all appropriate management according to their hospital’s protocol. The execution of the scenario itself takes 8 to 12 minutes, which is comparable to the time allotted to safely assess the emergency during labor to avoid adverse outcomes. Once the OB vendor has completed the scenario, they are immediately given a grade to summarize how they reacted to the situation. A debrief is provided highlighting what could have been done better.
Co-developers, Health Scholars, have partnered with NYC Health + Hospitals due to the Municipal Health System’s comprehensive, high-tech approach to OB simulation courses. By combining Health Scholars ‘expertise in VR technology and voice technology with NYC Health + Hospitals’ expertise in OB training, both groups have developed an engaging approach to skills training that will facilitate building clinical skills in healthcare systems. all over the country. The simulations are also configurable so that healthcare organizations can teach and validate skills against the evidence-based national guidelines of the American Academy of Obstetrics and Gynecology (ACOG) and California Maternal Quality Care Collaborative (CMQCC).

There are significant racial and ethnic disparities in pregnancy-related mortality. According to the Centers for Disease Control and Prevention, black patients have a pregnancy-related mortality rate about three times that of their white counterparts. The first scenario available to OB providers will simulate postpartum hemorrhage, which is the leading cause of maternal mortality. Additional scenarios, such as maternal code, shoulder dystocia, and fetal heart monitoring will be developed and added to the VR curriculum in the coming months.
The cast depicted in the VR scenario includes a pregnant black patient, partner, and diverse care team. Previously, OB simulations performed by NYC Health + Hospitals used specially designed, high-tech, full-body color mannequins.
In 2018, NYC Health + Hospitals maternal mortality rates were below the New York State rate of 20.9 / 100,000 live births.
